This form is to be completed when requesting a new index be created. Requests for new index codes in anticipation of any upcoming budget cycle should be submitted by the end of the calendar year.

Please attach backup documentation wherever possible. If you have already discussed the setup or obtained pre-approvals from Financial Resources, attach related documentation to this form.

*If an internal transfer needs to be completed, an Internal Transfer (Journal Voucher) request form must be completed in a separate step once you have been provided with the new Index number.
